Digital Maternity Records: My Pregnancy Notes
From 14 April 2026, the majority of paper maternity documentation at Dartford and Gravesham NHS Trust will be replaced with digital records.
This means that maternity service users booked in with the department after this date will be able to record and access their maternity information via the digital patient portal, My Pregnancy Notes. This will reduce reliance on handheld paper notes and promote greater accessibility.

Access and use:
You can access your digital maternity notes using the My Pregnancy Notes platform, using the link My Pregnancy Notes
You can discuss this with your midwife during your appointment, and they will advise on the support available.
Yes, you can request a paper copy be printed by your midwife when attending appointments. The midwife may not be able to give a copy immediately dependent on the facilities available where the clinic is being held.
You will keep your paper notes and continue to bring them with you to appointments. You will also be able to view your notes using the My Pregnancy Notes platform from 14th April.

You can share your maternity notes with anyone you choose by using My Pregnancy Notes, and this permission can also be removed at any time.
Your My Pregnancy Notes is protected by a password which you can change at any time.
Care, Continuity & Appointments:
If you go into labour somewhere else, you can share your maternity notes with clinicians managing your care using My Pregnancy Notes. You can also tell the hospital providing care to contact DVH and the maternity team can securely send a full copy to them via NHS.net email
You may still have some information that remains on paper, so please ensure to bring this with you to appointments.
You will see any changes your digital notes using the My Pregnancy Notes platform as soon as it has been saved by the midwife.
Interaction with your care team:
You can use the My Pregnancy Notes platform to communicate with the DGT Midwifery Team, but please note this is not a managed service, and is not to be used in emergencies.
This can be raised with your midwife at your next appointment where it can be rectified.
Sharing & Legal Rights:
Your maternity notes will remain visible on the My Pregnancy Notes platform, or you can still make a request via the route set out here: Health (Medical) Records :: Dartford and Gravesham NHS Trust
If your baby is discharged with you/to your address the baby records within the system will be available. If the baby is being fostered/adopted, you will not have access to the baby records via My Pregnancy Notes.

Practical Concerns & Experiences:
Anything that is documented within the system is available, currently pathology results do not go directly into the system, however if your Obstetrician/Midwife notes the results in the system, you will be able to see this.
Documentation is available once the clinician has completed the notes at the appointment. The clinician is not required to upload anything extra - it is an automatic process.
